Shockwave therapy is a relatively new treatment option in orthopedic and rehabilitation medicine. The effect of shockwaves was first documented during World War II when the lungs of castaways were noted to be damaged without any superficial evidence of trauma. It was discovered the shockwaves created by depth charges were responsible for the internal injuries. This created a great deal of interest and research into the biological effects of shockwaves on living tissue. The first medical treatment developed from this research was lithotripsy. This allowed focused shockwaves to essentially dissolve kidney stones without surgical intervention. Today, over 98% of all kidney stones are treated with this technology. The use of shockwaves to treat tendon related pain began in the early 1990s.A clinical shockwave is nothing more than a controlled explosion that creates a sonic pulse, much like an airplane breaking the sound barrier. The primary effect of a shockwave is a direct mechanical force. The exact mechanism by which shockwave therapy acts to treat tendon pathology is not known. The leading explanation is based on the inflammatory healing response. It is felt the shockwaves cause microtrauma to the diseased tendon tissue. Background
Shock waves have been in use for medical purposes since the 1970s. Originally developed for treating stones in the urinary tract and salivary gland, shock waves were later used for treating soft tissue injuries in the 1990s. How it works
A radial shock wave is a low-medium energy pneumatic wave generated by an air compressor inside the device. At Narrabeen Sports & Exercise Medicine Centre we use radial shock waves which have a lower peak pressure and longer rise time than extracorporeal or focused shock waves. Radial shock waves have the benefit of being lower energy, hence less painful, and the radial nature of RSWT emission means the waves can propagate over a wider area of the body.
When applied to musculoskeletal tissue, the shock waves have been shown to stimulate the release of tissue growth factors, new blood vessel formation and stem cell activation, all of which promote tissue regeneration. Substance P, the messenger in pain nerve fibres is also depleted, resulting in the nerves being unable to transmit pain for a short period.
The treatment
Each treatment involves application of the hand-piece of the device to the affected tissue and the transmission of shock waves. We recommend a minimum of 3 treatments, approximately 1 week apart.
